Perceptions, barriers, and practices regarding syphilis among women in rural Paraguay: a qualitative study in vulnerable communities

Introduction: Syphilis remains a public health concern in Paraguay, with a higher burden in rural areas where women face multiple barriers to access information, timely diagnosis, and treatment. Objective: To explore perceptions, knowledge, and barriers related to syphilis among adult women in a rural community of the San Pedro department, Paraguay. Methodology: A qualitative study with a phenomenological approach was conducted using in-depth interviews with nine women residing in General Elizardo Aquino, during February and March 2025. Data were analyzed through thematic interpretation and manual coding. Results: Limited knowledge about syphilis, multiple barriers to accessing health services (distance, lack of supplies, fear of judgment), and strong stigma surrounding sexually transmitted infections were identified. Participants proposed inclusive educational interventions and active male involvement as prevention strategies. Conclusion: Rural women interviewed face significant structural, cultural, and gender-related challenges in syphilis prevention and care. Strengthening health education, ensuring access to diagnosis, and reducing stigma through community-based approaches are recommended.
